ADVERTISEMENT TO BID LEBANON BUISNESS PARK STREET REHABILITATION PROJECT CITY OF LEBANON, INDIANA Sealed proposals will be received by the Lebanon Redevelopment Commission, Owner, at the Clerk Treasurer's Office at One Municipal Plaza, 401 South Meridian Street, Lebanon, Indiana 46052 until 3:00 p.m. (local time), on September 12, 2016, and then publicly opened and read aloud at the Lebanon Redevelopment Commission meeting at 6:00 p.m. (local time). Any bids received later than 3:00 p.m. (local time) will be returned unopened. DESCRIPITION OF WORK: Phase 1-Garoffolo Blvd.-Mill and fill 1320 feet by 28 feet width from 425 N. Mt. Zion Road to the end of curb. Mill 4" deep of existing asphalt Pave 3" with #8 binder Finish with 1 ^" #9 Surface Maintain Traffic, Flagman, Barrel, and Signage-Open to Traffic Phase 2- Mt. Zion Road-Mill and fill 1670 feet by 24 feet width from the end of the curb to the T-East of Maplehurst Profile Mill 2% Pave 1670' x 24' Mill 5" of existing asphalt Pave 4" with #8 binder Finish with 1 ^" #9 Surface Excavate 1' deep X 2' Wide for shoulder 2' stone shoulder with compacted #53 stone Maintain Traffic, Flagman, Barrel, and Signage-Open to Traffic Phase 3-North Mt. Zion Road-Mill and fill 2280 feet by 24 feet width to Enterprise Blvd Profile 2% Pave 2280' x 24' Mill 4" of existing asphalt Pave 3" with #8 binder Finish with 1 ^" #9 Surface Maintain Traffic, Flagman, Barrel, and Signage-Open to Traffic Center Line will be Double yellow Thermoplastic 4" Line will be thermoplastic 4" solid white on each side Stop bars will be white thermoplastic 24" BID DOCUMENTS: Bids shall be accompanied by a completely filled out State Board of Accounts Form No. 96 (Revised 2013) and acceptable Bid Security. Any bid not accompanied by the above-required items shall be deemed to be a non-responsive bid. No Bidder may withdraw his proposal within a period of 90 days following the date set for the receiving of bids. The Owner reserves the right to retain any and all bids for a period of not more than 90 days and said bid shall remain in full force and effect during that time. The Owner further reserves the right to waive informalities and to award the Contract to any Bidder all to the advantage of The Lebanon Redevelopment Commission or to reject all bids. BID SECURITY: Each bid shall be accompanied by an acceptable certified check made payable to The Lebanon Redevelopment Commission or an acceptable bid bond in the amount equal to 10% of the total bid price executed by an incorporated surety company in good standing and qualified to do business in the State of Indiana. BONDS: A Performance Bond and Payment Bond each in the amount of 100% of the Contract price will be required from the Contractor to whom the work is awarded. INDIANA REQUIREMENTS: Standard Questionnaire Form 96 (Revised 2013), completely filled out and signed, including attachment of Contractor's Financial Statement.
